It's not a bug, but a feature.Actually, the reason why he spawns there is he should spawn in the fore... So at the end of the grotto, there is a rock that lights up with the witcher sense. Its got a symbol that looks like a swirly sun. I'm assuming there is a way to get thru it, but i cant figure it out? Anyone know how to?

This is very area specific, you need to be at dragonslayer grotto run around circle around to the left side of the mountain from the fast travel board and you'll find a leshen in the woods. You want to avoid taking it hits and slowly lead it back to the side of the mountain near the entrance to the grotto then book it inside.
